Placing an Order
We sell only to hospitals, clinics, and health care professionals.
You can place an order online, or by phone, fax, or mail.

PO Number / Payment
Orders from hospitals and large clinics must be accompanied by either a PO (purchase order) number or prepayment. Smaller clinics and practices may be invoiced. We also accept Visa and Mastercard.
Quantity Discounts
To determine your discount price, add up the number of items from each group (A or B), then use that number to find the price per item (see table below). The more items you order within a group, the lower the price per item. Group A items are priced by the booklet. Group B items are priced by the pack; each pack contains 50 pamphlets.

Prepayment Discount
A 2% prepayment discount is given on orders received by mail with a check enclosed. Credit card orders do not qualify for the prepayment discount.
Shipping & Handling Charge
The shipping & handling charge is 7% of the order subtotal, with a minimum of $5 and a maximum of $50.
Sales Tax
Sales tax applies only to California orders, and is calculated at 9% of the order subtotal. You may modify the calculated tax on your invoice if it is different than the actual sales tax in your area.
